By doing this you consent to your images, name and instagram handle being shared. Any pictures provided will only be used for this story series. The story series may be saved in the highlights and shared on the Feed Eat Speak website in the form of a blog. They will not be used anywhere else unless you are asked for your explicit consent. You responses to the form may be edited but nothing will be added.
If you wish to remain anonymous please send non identifying images and give a false name for yourself and baby - do not include your instagram handle but do please also give us your full name and email address so that we can correspond with you.
